The Lineage Form is a service for HFA Members only (or those who are in the process of joining). Sumitting a Lineage Form is strictly optional and not required. If a Member chooses to submit a Lineage Form, it is reviewed by the HFA Genealogy Committee versus information already in our files. In some cases we can extend submitted lineages several additional generations. We will also provide more details or additional information if available, correct any info that we know to be incorrect, and offer suggestions for further research. Please keep in mind that we are a non-profit association run by un-paid volunteers. We are happy to help Members with the information we do have, but we have neither the time or resources to do original genealogical research on your behalf.

Note: Due to constraints of time and resources we do not responds to Lineage Forms for non-members (membership info is available here). Non-members, however, are welcome to submit queries using this query form.

Here’s a fictitious example of an entry for a single generation:
NOTE: For people in your lineage who are still living, please do not give us the exact month and day of their birth or marriage. At most, tell us the year and place they were born or married. If for some reason we find that we need more info in order to review your lineage, we will get back in touch with you as needed.
Submissions Policy
All submissions to the HFA of any kind
become the joint property of the submitter
and the HFA, with the understanding that the
HFA may, using its own discretion, share this
information with others unless otherwise
requested in writing by the submitter.
The submitter’s expenses cannot
be reimbursed by the HFA.
